[minicoredumper] moved to github

Hello,

The Linux Foundation will be shutting down its git services for
workgroups, so the DiaMon Workgroup (of which minicoredumper is a part
of) has setup a new account on github. The git repository and bug
tracking has moved there:

    https://github.com/diamon/minicoredumper

    https://github.com/diamon/minicoredumper/issues

I have updated the website to point to the new links. I also updated the
documentation within the repository.

Thanks to the Linux Foundation for hosting this project for the last 5
years!
